Appropriate Permission Slip
Trips away from normal meeting place need a Caregivers Permission for Girl Scout Activities Form filled out. Overnight trips need a Caregivers Permission for Trip/Camp Activity Form completed.
Keep the following forms with you:
Health History Form and appropriate
Fully Trained Volunteers; be sure to meet the girl/adult ratio for both age level and activity. Refer to chart for specifics.
Certified First Aider; be sure to bring your individual first aid kit on all trips.
Specialized Trained Volunteer when the trip/activity requires it. (ie canoe instructor)
This online form can be used for day trips and single overnight trips that do not involve high risk activities. Submit a minimum of 5 days in advance of trip. If you do not meet the 5 day requirement, please CALL SU Director Susan Ramsland.
When preparing for any activity with girls, always begin with the Safety Activity Checkpoints (SACs) written specifically for that particular activity.
